Two refinements to the Session Reference Shelf (follow-ups to #246/#240):

Quick-switcher wiki hits open in a new tab — selecting a wiki entry in the Ctrl-K palette currently navigates away from the shelf. Open it in a new tab (window.open) so the live shelf stays put during play. (Shelf-card hits still stage in place.)
Auto-refresh the board — poll the shelf and refresh on tab focus/visibility so a card added from another tab (Workbench/wiki "Add to shelf") appears without a manual reload. Merge so adds/removes sync without clobbering an in-progress drag or unsaved notes.
