Phase 3 scope is intentionally loose. Per the roadmap, do not commit to this scope until Phase 2 is stable and there is operating experience with the Phase 1/2 surface.

Scope

Extend the pf2e adapter with journal_to_entry(lore_entry).
Add a "Push to Foundry" action on lore entries that writes the entry to a Foundry journal page.
Hide the action when the adapter returns None.
